do you know what kind of video card you have? either go to the device manager and look under the display adapter area or go to start>run> type in "dxdiag" (without quotes) and look under the display tab in there. your settings seem fine but this could be related to your video card. you might want to try the game on either the other version of epsxe (1.5.2) or another emulator like psx or pcsx. have you tried running the game from both the disk and an image? also does the game lock up totally or does it just have that graphical glitch shown in that screenshot? these are my settings in case you want something to try:

Plugin: Pete's DX6 D3D Driver 1.1.76
Author: Pete Bernert
GFX card: NVIDIA GeForce Go 7900 GS

Resolution/Color:
- 1920x1200 Fullscreen - [32 Bit] (ignore the res.)

Textures:
- R8G8A8A8
- Filtering: 4
- Hi-Res textures: 0
- VRam size: 256 MBytes

Framerate:
- FPS limit: on
- Frame skipping: off
- FPS limit: 60.0

Compatibility:
- Offscreen drawing: 3
- Framebuffer texture: 2
- Framebuffer access: 4
- Alpha multipass: on
- Mask bit: on
- Advanced blending: hardware

Misc:
- Scanlines: off [0]
- Unfiltered FB: off
- Dithering: off
- Screen smoothing: on (optional)
- Full vram: on
- Game fixes: on [00000a01] (enable any game-specific fixes, otherwise ignore this section)
