How Can I Convert Save Files from Actual PSX Memory Card to use with Emulators

Hello,
I'd like to know how to convert PSX save files from actual memory card so I can use them with emulators (and preferably vice versa). I'm able to transfer files on the memory card to my computer via a PS2, and each save "file" exist as individual folders with one file without extension (presumably the save file), an icon, and another small file. I've looked up quite a few PSX save conversion tools but none of them seem to work.

Thanks.
