Actually, to be specific, that's デュープリズム, "Dewprism", the Japanese version. The name was changed to Threads of Fate for the US.

Yes, the game is Dewprism (Threads of Fate), what really floored me was that I keep seeing the picture and it was striking me as odd, though I couldn't figure out why. Then I realized what I was looking at, I was just playing Threads of Fate, with the actual disc in my drive xD;
