USA version, this game works runs perfectly besides the sound. The bouncing Dreamcast logo sounds perfect, but after that nothing. If I start playing the game it's silent, but if I let the demo roll the mice start squeaking constantly and there's constant stuttering chimes that I assume are part of the 'begin round' jingle(not an audio track). If the demo runs a second time the squeaking will continue endlessly, into the Sega logo and the menus, and even the next round.

I tested the PAL version and it's completely silent.

For what it's worth, the system menu sound effects are similarly wonky.

I changed my buffer size to 2048 in the cfg file and it affected nothing.

My specs are (more than enough):
Windows 7 64-bit
Intel Core i7 CPU Q740
700GB HD
6GB RAM

Which version of nullDC are you currently running? If its an older version, such as beta 1.6 or v1.0.3, then you may want to consider upgrading.

I was on 1.0.3, so the update definitely helped. Music is fully functional. Sound effects are better, at least they all play but they're very broken. Hovering over a menu option, mouse squeaks, cat explosions, etc. have around 1 second lag. The menu selection chime takes about 16 seconds to complete as it painfully executes every single note, overlapping itself I transverse multiple menus. In game jingles have similar problems. NTSC and PAL versions perform identically (bar framerate)

This version of NullDC ships with optimized buffer size settings, so that was left untouched for now.

Well, I'm glad that at least something got better. I recommend that you report this issue on nullDC's Google Code issue tracker so that the devs can hopefully fix this problem in the future.
