It runs up until after the opening movie, then epsxe crashes and gives me an error that says "Opcode 2f UNK (PC 00088a24) (2009.16). I have tried everything I can think of to get it running, but it won't run on any version of the emulator. Also it used to run before, but with all the menus blacked out from the status screen. This is the only game epsxe has problems with.

That's a CD-R read-error (issue), try another CD-R plugins, it'll fix it.

The thing is, I'm not running it from the CD, I'm running it from an ISO.

Then try running it from a CD.
Or make a new CD Image.

I tried changing plugins and running it from the CD, still gives me the same error. The CD works fine on a regular playstation and has no scratches on the bottom.

1- Post a screenshot, it may help to figure out what is the real problem.
2- Also post your system/emu+plugins specs.
3- Did you ever succeeded running the emu with other games ?

the drive you are using doesn't support the required read mode.

Then... how about making a pretty Image of the disk, and then mount it on a Virtual Drive?

Tried mounting the ISO with daemon tools, then configuring the CDROM plugin to run from that, but it didn't work. Also, every other game I try works fine, this is the only one that refuses to work. Here is a screenshot of the error when I try to run it from the CDROM.

Which GFX plugin are you using ?
PS : You may need a ppf file for your game to work. i don't know.

I'm using pete's openGL driver 2.8. And what's a ppf file?

Try a D3D version.
And what's a ppf file?
I don't know for sure, but i think they're files that allow emus to bypass some extra-read-protection or somthing
PPF FILES ON NGEMU (http://www.ngemu.com/misc.php?action=ppf)

Well I don't see one for FF4 on that list, so i assume it doesn't exist.

Tried the D3D idea, no dice. I'm beginning to think it's a faulty disc image, though I don't know how that could happen.

yes they get around the copy protection most PAL region and a few NTSC region games have.

You mentioned it worked before. What was your setup by then? And, what has changed?
By that, if you didn't know about ppf, you probably don't need one.

Dude post a screenshot of your emu while doing the crash trick.

Ok so I just tested it on 1.6.0, and it loads. However, whenver I press triangle (or the equivalent on a gamepad), the menus in the status screen are blacked out. Also in the opening sequence, when Cecil fights some monsters on the airship, a lot of letters are missing from the dialogue. How can I fix this?

Use PEOpS Soft plugin? It would be a better choice for FF4.

Thanks a bunch, finally got it working right with the PEOpS Soft plugin.

amusingly, the peopsoft plugin runs like ass for Tactics, and the OGL2 plugin works perfect.
