Just a hunch... what is the drive letter of your CD-ROM drive?

Just as I thought.... a similar problem has been reported before, and here's what Juan (one of the authors) said:

Ok, try changing your cdrom default drive letter (I or J), from your system hardware configuration, forcing it to use a letter between D and H.
Then re-boot your system, check that the cdrom drive letter has changed and re-config your cdr plugin.

As Kairi00 said that's the possible solution. If you need more info, just browse the Nezzar post "Emulation not starting at all".
The AdriPSX ILE 1.0.4 release (coming really soon) will not have this problem.

See ya.
