Huh, patch?
If you were talking to me... No, it happened on PS2, well with "MY" PS2 which had some kind of problems moving the deck where the laser is placed, fixed that by cleaning and oiling it... So for that reason the game stopped for a while during the dressphere change and then it never really finished it and it came out "naked"!
Happened only once so I guess its not very common
