Happens to me too, using gsdx (dunno if it's pcsx2 or gsdx fault)
If I use ALT-F4 the emu interface disappears, but pcsx2 is still running in the background (this is probably why it gives problems if you try to rerun it, because the 2 instances of the program probably conflict)
If I press esc, a black screen appears, but I cant use alt-tab or the win key to ecape it.
The solution I found is: press ESC to bring up the black screen, then press ALT-F4 once to close the emulator. This works for me, even if it still gives me a crash sometimes (rarely thou...)
